We set a very high standard for our care and we are very proud
of the treatment that we have provided for you.
We will guarantee your restoration for a period
of five years. This guarantee ensures that if your restoration
needs to be replaced within five years from the treatment
date, we will replace it at no charge to you. If additional
treatment is required on the tooth, we will credit your payment
toward the recommended treatment. In order to maintain this
guarantee, all of the following provisions must be satisfied:
- You maintain the recommended schedule for preventative
appointments. This includes cleanings, fluoride treatments,
routine dental examinations and x-rays.
- If diagnosed with periodontal disease, you accept periodontal
treatment and maintenance recommendations.
- If you develop any problem that should result in the replacement
of this restoration, the cost of the original service will
be credited to the cost of the new procedure(s) provided
the new services are performed in this office.
Our guarantee is limited to this restoration only and does
not include additional procedures such as, but not limited
to, endodontic treatment.
